The stage is set; the lights are bright! Welcome all to TxEVER: A Star is Born!The 64th Annual Texas Vital Statistics Conference hosted by the Department of State Health Services Vital Statistics Section. This conference will equip statewide partners, registrars, and superusers with essential comprehensive trainings, empowering focus group discussions, live demonstrations, and much more TxEVER-focused content.

You are the star of the show! Attendees can choose from a variety of learning tracks, including core how-to registration trainings, hands-on DIY time, and live TxEVER demos. The conference provides exciting roadmaps to new statewide collaborative initiatives and campaigns fostering engagement and inclusion for both superusers and local champions.

The three-day conference takes place on December 10-12, 2018, and is co-sponsored by the Texas Public Health Association. It's Showtime!

By submitting a registration form, you are consenting to your picture being taken during the Vital Statistics Conference and used for marketing promotional purposes.

Registration: $300 must be paid in full no later than November 30. Contact TPHA for more information about registration at txpha@aol.com

Registration fee includes scheduled breaks, breakfasts (Tuesday Breakfast Buffet and Wednesday Continental Breakfast), access to exhibitors and all conference materials and give-aways.

Failure to Show:

Failure to show does not release you from the responsibility to pay the registration fee. If you are unable to attend you are required to notify TPHA at txpha@aol.com prior to the first day of the conference.

Substitutions:

If you register but are unable to attend, another representative from your agency may attend in your place. Please email written notice of substitutions to to txpha@aol.com by December 1, 2018. Refunds will not be issued and agencies will be billed for all submitted registrations.

Refunds:

No refunds after November 9th. Refunds granted prior to November 9th are subject to a $50 service charge.
